Family Promise Rochester's Coordinated Entry Navigator is a key component of the River Valley Continuum of Care (CoC).  The Continuum of Care is a community planning group that coordinates services and maximizes resources to reach the goal of preventing and ending homelessness.  The River Valley CoC is a planning group made up of the 20 counties that span across the SE MN region.  

Coordinated Entry is an activity of the CoC that makes sure all people experiencing a housing crisis have fair and equal access to housing.  Individuals, families and youth will be identified, assessed for, referred, and linked to housing and services based on their individual needs.  

The purpose of Family Promise Rochester's Coordinated Entry Navigator is to help participants experiencing homelessness gather documentation support by establishing homeless history, disability documentation, income and other necessary verification in an effort to obtain stable housing.
